Abstract

Community and nursing interventions in healthcare are vital to the reduction of disease and disability, the promotion of health and wellness, and the lowering of overall disease burden in health. This paper examines how nursing interventions integrated with community healthcare support systems work as a structure, with the focus on primary health teams, health education initiatives, vaccination programs, chronic disease management initiatives, and home health monitoring systems, in producing better preventive health, earlier disease detection, and lasting health results. In both urban and rural contexts, it has been demonstrated that nursing interventions in communities that support proactive measures lead to decreased hospitalization and reduced healthcare expenditures, and increased trust and responsibility from the public towards healthcare delivery. By reviewing the operational plans, intervention models, and outcome measures, it demonstrates a unique correlation between the community health support structure, nursing support practices, and improvement in population health over time.
