This paper describes the development and implementation of clinical care models for the effective management of non-communicable diseases (NCDs). NCDs, such as cardiovascular disease, diabetes mellitus, chronic respiratory disease, and cancers, are the leading cause of morbidity and mortality around the world. The growing number of NCDs is putting pressure on healthcare systems, especially in low- and middle-income countries. This paper describes the process used to develop clinical care models that respond to the individual risk-factor specificities encountered by individuals with NCDs, applying evidence-based interventions, a patient-centered approach, and a multidisciplinary healthcare team. It will also describe how clinical care models were implemented in a healthcare system, challenges, stressors, and successes of incorporating new models into existing systems. Components of the models included prevention, early identification, individualized treatment plans, and therapeutic strategies for long-term management. We contribute to the argument that by focusing on the healthcare professional, specifically nurses and physicians, we can provide an overarching framework within clinical care models to enhance patients' experiences and more effective outcomes, while acknowledging the case for reduction of the burden of NCDs in population health. We suggest that clinical teams together can reduce the burden and improve the health of populations through the adoption of comprehensive clinical care models.
